1. What is the 8915-D form?

The fillable 8915-D form is sent to the IRS by individuals who want to clarify retirement plans, repayments, distribution. It is also known as the disaster retirement plan distributions and repayments form. If you are affected by any disaster listed in Table 1 of the document, and you received a distribution, that is considered to be the tax treatment.

2. What I need the 8915-D form for?

  • Download the 8915-D form if you received the disaster distribution from the retirement plan. This distribution must be made in 2019;
  • The form is used for stating that you got the qualified distribution for the construction or purchase of the house in the area listed among disaster areas of 2019. If you made repairs partially or completely, you have to file the form. All this started construction business must have been done before June 17, 2020.

  • Part 3 is about disaster distribution of 2019 from SEP, Traditional, Roth, Simple IRAs;
  • Part 4 is about the construction or purchasing the main house in the disaster areas;
  • Don’t forget to sign it and put the date.
