How To Fill Out Form 8868 Instructions
Part I: Automatic 3-Month Extension
Provide the organization's name, address, and EIN. Include the suite, room, or other unit number after the street address.
Enter the tax year for which the extension is requested.
Check the box for the form for which you are requesting an extension.
If you have a balance due, enter the amount. However, most forms, like 990, do not require tax payments.
Part II: Additional (Not Automatic) 3-Month Extension
Use Part II only if you have already been granted an automatic 3-month extension on a previous Form 8868 and you need an additional three months. You must show reasonable cause for this request.

The form must be signed by the authorized individual (an organization officer, owner, etc.).
