What Is ACORD Form 28?

ACORD Evidence of Commercial Property Insurance is a written confirmation that the insured person actually has valid insurance for the property used for commercial purposes. Most often, it is provided as a stand-alone document. To fill it out, you need information from the insurance certificate.

What Do I Need the ACORD Form 28 For?

  • Fillable ACORD 28 is completed and provided if the insured person requires a pledge or a loan for commercial real estate.
  • This blank is only used concerning property insured under the Commercial Lines policy and when a lender needs more details about insurance. Otherwise, Forms 24 or 27 should be used.

How to Fill Out ACORD Form 28?

It is a simple one-page printable file with four main sections:

  • Indicate the date of filling out the document and enter information about an agency/producer and insured(-s), as well as data about the latter's company. Also, here you need to enter information about the policy: type, number, loan number, and the expiration date.
  • In the next block, you should provide detailed property information. As a rule, this one field is enough for the full address, but you can additionally use Form 101.
  • In the "Coverage Information" section, all the cases covered by insurance are presented (terrorist attacks, floods, fungus, etc.). If something does not apply to the property being described, please tick the "No" box. Be sure to include the relying limits.
  • The "Additional Interests" section should be completed if applicable. Choose the interest type, and specify names and addresses. An authorized representative should sign the file.

When done, email the PDF document to a recipient, share it via a link, print it, or save it locally.
